Single Person Will

Enduring Medical Guardianship
A enduring medical guardianship (power of attorney) is a legal document where you appoint someone to make medical treatment decisions for you. The appointment only becomes valid when you are unable (i.e. unconcious as a result of an accident) to make medical treatment decisions for yourself. An enduring medical power of attorney cannot be used to make financial, legal or lifestyle decisions.
